A forklift was lifting a double stack of trailer fenders when the top pallet of the stack fell onto the forklift. The pallet broke and trailer fenders fell onto the injured employee, who was standing nearby supervising the operation. The employee sustained a broken nose and two broken ribs, requiring hospitalization.
