Incident Narrative
An employee was walking on stilts to his working area. He was working in a computer room (raised floor) that was covered in tile squares. Someone had removed a carpeted tile and he fell between the false floor and carpeted sections of the floor, dislocating his left elbow and breaking his right wrist.
