Incident Narrative

An employee was performing pipe-fitting operations at the top of a 6-foot rolling staircase (rollers locked in place). When she reached out and grabbed a fitted pipe, it came loose and caused her to lose her balance and fall down the stairs. She broke her right wrist, left hand, and sacrum.
