Incident Narrative

An employee entered a trash cart as part of a process where a forklift operator would raise the cart off the ground so the employee could shovel trash out of the cart and into a compactor. The metal cart slid on the forks toward the forklift while the employee was holding onto the edge, and the employee's left middle finger was amputated when the trash cart made contact with the forklift apron guard.
