Incident Narrative
An employee was replacing the bearing of a poultry elevator and separator using a hammer and wrench. When he hit the sprocket to gain access to the bearing, the sprocket came loose and caused the elevator to fall. The employee jumped back and the force from landing broke his right foot and lower right leg.
