Incident Narrative
An employee was going to install bolts onto plates on furnaces located on the third level when he stepped on a piece of metal stair grating that had not been welded down. The grating flipped and the employee fell through the opening to the stair landing 7 feet below, suffering a fractured L1 vertebra.
