Incident Narrative

An employee was spray painting while standing on top of an approximately 8 foot x 15 foot rectangular tank that was approximately 7 feet tall. There were five 2 foot x 3 foot holes cut out of the top surface of the tank that were not covered. The employee fell into one of these holes and suffered a fractured pelvis.
