Incident Narrative
On October 24, 2016, at approximately 3:00 p.m., an employee was attempting to move a shutter head assemble from the shutter head machine on Line 16. The equipment was stuck. The employee went to get help and the two employees jogged the piece and the 40 pound shutter head fell approximately 18 inches. While the equipment was falling, the employee tried to catch it and his left middle fingertip was caught between the equipment and wood board resulting in amputation of the fingertip.
