Incident Narrative

At 6:15 a.m. on July 8, 2022, an employee was preparing to weld hinges and other parts to an inner trailer door. The employee was picking up the top hinge part of the trailer door with a pry bar, as well as placing a small block under the inner door or top hinge part. The pry bar slipped and the inner door fell about 4 inches, landing on the employee's left little finger. The finger was broken and the fingertip was amputated.
