Incident Narrative

An employee was transferring small boxes that were inside a large wooden crate that was stationed on a shelving unit about 18 feet up. The employee was using a mobile platform ladder to reach the boxes. A coworker was operating a forklift and had raised a pallet to where the employee was working. As the employee was transferring the small boxes onto the pallet, the wooden crate became dislodged from its current position on the shelving unit. When the employee attempted to straighten the wood crate, he lost his balance and fell, first hitting the pallet that was raised and then the ground below. He suffered a compression fracture and lacerations to his head, requiring hospitalization.
