Incident Narrative

Employees were using an overhead crane to lift steel plates from the bed of a truck. An employee attempted to attach chains to offload the steel plates from the truck. As he was lowering the steel plate onto the floor, the plate began to move. His right hand was on the plate and his middle finger was caught between the lip of the plate and the hook, amputating his fingertip.
