Incident Narrative

An employee was pumping oil into a production line via a hot oil truck. Oil began to spray out of the pipe and ignited the burner. The ensuing fire engulfed the employee's upper body and he received third degree burns, requiring hospitalization and surgery. PPE was not worn at the time of the incident.
