Incident Narrative

An employee on a second-level platform was opening a slide gate at a height of approximately 14 feet to place a pallet on the deck. The slide gate was 4 ft H x 12 ft W on the exterior wall, attached by a sliding mechanism. The employee fell to the ground along with the gate, suffering multiple fractures to the femur, pelvis, ribs, right leg, and arm.
