Incident Narrative

On 9/14/16, an employee was transferring cleaning chemical from a 55-gallon drum into a larger tote when the chemical transfer pump blew a hose off the hose bar and sprayed chemicals on the employee's face, arms, and torso. The employee was hospitalized with chemical burns.
