Incident Narrative
An employee visually determined that a metal flange on a transfer had become disconnected from a metal plate, allowing a transfer roller to fall. When he touched the flange, the metal plate became loose and smashed his right ring finger between the metal plate and flange. The transfer was locked out at the time.
