Incident Narrative

An employee was cutting a flat piece of cork stock (1/8" thick from a 12"X12" section) to be used as trimming on furniture. He was having difficulty cutting the cork material because it would not slide on the table saw surface due to the cork material adhering to the table. He proceeded to apply more force, and as he attempted to push the stock into the blade, the material inadvertently folded, causing his left hand to move into the rotating blade. His left-hand pinky finger was amputated, and his ring finger was lacerated.
