Incident Narrative

An employee was helping a coworker change out a plug catcher. After re-inserting the screen, the employees attempted to attach the connection. As the connection cap was pushed forward to prepare it for threading, the loose bolts on the cap allowed for significant play and crushed the employee's left thumb between the cap and flange bolts. The employee's left thumb was partially amputated below the fingernail.
