Incident Narrative

A 4'x11' concrete form wall was being lifted with a crane. During the lift, an employee walked near the lift area to retrieve some tools. As the employee walked into the lift area, the form wall got caught on another piece of the construction project. The form wall came out of it's rigging and fell to the ground, tipped over, and struck the employee on the head and back. The employee was hospitalized and suffered 2 fractured vertebrae.
