Incident Narrative
An employee operating a forklift retrieved a pallet from the metal rack in the meat department. There were two pallets on the rack; one pallet was leaning on the other. When the employee placed one pallet on the floor, he exited his forklift to check the load. Fifteen to 20 small boxes from the pallet on the rack then fell and struck the employee, fracturing his left leg below the knee and three toes on his left foot.
