Incident Narrative

On or about 7/1/16, at approximately 11:30 a.m., an employee was climbing into an articulating boom lift from the horizontal section of a structural steel tube that was 12 feet above the ground when he began to slip. A coworker who was in the lift went to grab the employee and was pulled out of the lift, causing both employees to fall 12 feet to the ground below. The employee was hospitalized overnight for a fractured vertebra. The coworker was not hospitalized.
