Incident Narrative

A crew was pulling tubing out of a hole. As elevators were coming down, an employee grabbed the tubing elevators by the handles and was unable to push them back far enough to latch them onto tubing. The elevators hit the top of the tubing collar, causing it to shift and pinch his right index finger between the elevators and tubing bail. His finger was lacerated and fractured.
