Incident Narrative
An employee was shoveling gravel into a 1-foot trench by a partition wall. The site supervisor was operating a mini excavator near the partition wall while the employee was working on the opposite side. The mini excavator bumped into the wall, causing it to fall to pieces. Some of the pieces fell on and broke the employee's hip.
