Incident Narrative
An employee attached a strap from the excavator bucket to the jack hammer and then signaled for the operator to move the bucket back. The employee saw the strap start to get tighter when the jack hammer popped up and crushed his right hand between the hammer and the pipe. His right pinky finger was amputated.
