Incident Narrative
An employee was setting up concrete cylinder targets on a gun range. A forklift was pressing a metal plate against the cylinders to help position them tightly as the employee was using his hands to position them. His thumb was then caught between the metal plate and concrete cylinder, tearing some skin off his thumb tip and requiring surgical amputation.
