Incident Narrative
An employee was moving a glass rack with a pallet jack. While lowering the rack of glass, the employee had the handle of the pallet jack in a vertical position. When the rack was lowered, the employee's thumb was caught and partially amputated between the rack and the handle of the pallet jack.
