Incident Narrative
On or about May 4, 2016, at 1:20 p.m., an employee was reaching to clean debris off of the laminator machine's chrome roll when the chrome roll turned and caught his hand between the chrome roll and the guide roll. His arm was wrapped around the chrome roll causing several fractures to the radius and ulna of his right arm. He was hospitalized for surgical intervention.
