Incident Narrative

On 04/01/2016, an employee was hospitalized due to lacerating his left forearm. The employee was using a pocket knife to cut a hard plastic banding off of stacks of boxes when the banding sprung and caused the knife to lacerate his left radial artery. The employee required surgery.
