Incident Narrative

An employee was helping to set up a trellis from a man basket being lifted by forklift. The basket sat directly on top of the forks, approximately 14-to 16-feet-high. When the employee moved to the end of the basket, the basket tipped and he fell out. The basket then landed on top of him. He broke teeth, fractured multiple ribs, broke his back, and suffered internal stomach bleeding. The basket was not secured at the time of the incident.
