Incident Narrative

An employee was cleaning a piece of equipment while standing on a 6-foot fold-out ladder. The employee placed his second foot back on the ladder and felt as if the ladder was going to tip over. To avoid falling on his back, he dismounted the ladder, landing on the ground and fracturing his right knee.
