Incident Narrative

An employee was working on a lift truck. While putting the chain back on the lift truck's forks, the employee accidentally knocked it in gear and the boom fell. The forklift chains on the boom amputated the two middle fingers on his right hand. One finger was cut below the nailbed and the other was cut in the middle of the nailbed.
