On or about March 16, 2016, at approximately 4:00 p.m., an employee was trying to clear a jam on a conveyor that was transporting cones on molds when an automated oven door came down on his hand and pinched his right middle finger, amputating the nail and fingertip. The equipment was not locked out at the time of the incident.
