Incident Narrative

An employee was changing grease fittings and lines on a stand in the plant. The employee was picking up the hoses as he was walking away (there is a channel that the profile gauge moves back and forth over). The profile gauge had been moved away at the time, uncovering the channel. The employee stepped in the open channel, which is 26" deep, and broke his left leg.
