Incident Narrative

Employees were standing in front of an extrusion machine studying a test run of material while other employees were grinding on the mold. A sudden release of pressure forced material and components out of the mold. As a result, an employee suffered multiple face fractures, a laceration above the right eye, broken fingers, and broken ribs.
