Incident Narrative
On February 11, 2016, an employee was performing a flare test on a 5" metal tube. The employee was holding the tube on the test cone and cycled the hydraulic press. The tube slipped off of the test cone, pinching the employee's finger between the tube and the bottom of the press. The employee's left pointer fingertip was amputated.
