Incident Narrative
An employee was hanging a coil of steel on a coil holder using a metal mesh sling. The sling was being removed when the crane eye caught the shaft of the mandrel. The employee's left middle finger was then caught between the eye of the sling and the mandrel, amputating the fingertip.
