Incident Narrative
A worker cut a 4-foot x 6-foot x 5-inch piece of concrete slab from the operating floor, which fell toward the barricaded area in the basement. As the concrete slab fell, it struck a piece of equipment and was deflected to an area outside of the barricade. The slab then struck an employee standing nearby, lacerating the employee's leg.
