Incident Narrative
An employee was emptying the dust collector on a fines hopper. After she centered the drum under the discharge, she raised her left hand to shake fines off of her hand. Her hand entered the vertical discharge chute, and her fingers contacted the rotating vanes of the rotary valve, amputating her left middle and ring fingertips.
