Incident Narrative

An employee was working in the graphite mixing department. The mixing process creates one pound graphite mix balls. Once the mixing process was completed, the employee dumped the contents of the mixer into a stainless steel box. One of the mix balls landed on the edge of the stainless steel box. The employee reached for the mix ball to throw it into the box. As the employee reached for the mix ball, he lost his balance and pinched his finger between the mix ball and the edge of the stainless steel box. The employee suffered the amputation of the left middle fingertip as a result of the incident.
