Incident Narrative
On January 30, 2019, two employees were attempting to remove a nut from the bolt of a blow-out preventer (BOP) stack with a torque-n-test. The BOP stack was lifting 1-2" with the pick up line connected to the block when the rigging shifted and came down on one of the employee's left index finger severing it at the second knuckle.
