Incident Narrative

Two employees operating stand-up reach trucks were rounding a corner at the same time and collided. One employee was traveling with the forks behind him and the other employee was traveling with the forks in front him. The truck traveling with the forks in front struck the other employee in the lower leg, causing a compound fracture to the tibia and crushing the left ankle.
