Incident Narrative

On November 17, 2015, at approximately 10:40 am, an employee operating a stand up forklift to transport tires from the back of the building to the Tire/Battery Center hit a bump and fell off the forklift. The forklift rolled over his foot. He was hospitalized with two broken toes and some damage to the skin.
