Incident Narrative
An employee was changing the force draft fan belt. He turned off the fan. Due to the inertia, the belt did not come to a complete stop and began to rotate. The employee's right index finger got caught between the belt and the pulley, and was amputated between the last joint and base of the nail. The machine was not guarded at the time of the incident.
