Incident Narrative
An employee was unloading equipment from a tilt trailer. A D-ring from one of the hold-down straps was caught between the trailer and the tilt bed. The employee tried to knock out the D-ring with a sledge hammer. When the D-ring was knocked out, the trailer bed came down and amputated the employee's right index fingertip.
