Incident Narrative

On or about October 22, 2015, at 3:45 pm, a temporary employee was injured when he fell approximately 12 feet from an upper level. The employee attempted to stand on a pallet that was on the forks of an elevated forklift when the pallet broke. He was hospitalized with injuries including broken wrists, ribs, a broken shoulder and a broken cheekbone.
