Incident Narrative
An employee was moving a 220 pound cutting edge (blade) onto a pallet using a forklift. The blade was unstable and the employee exited the forklift to steady the blade. The blade shifted and fell onto her left ring finger resulting in the amputation of the finger up to the first digit.
