Incident Narrative

On 10/21/15 at approximately 3:30 PM, an employee was cutting banding material off a stack of steel beams. The steel beams were 3 feet high and 20 feet long and came in stacks of two. They had wood blocking that held the two beams and steel banding material together. The employee was on the ground between two stacks of beams and was using a circular grinder to grind off the banding material. When he cut the last beam, the beams fell on him resulting in his hospitalization for a broken pelvis, right femur, and right tibia.
