Incident Narrative
Three employees had just retrieved a wooden beam from under a section of ductwork. One of them was bending over to pick it up when the duct section shifted and a piece of angle iron slid off it onto that employee's left thumb. The thumb was broken and the employee was hospitalized, undergoing surgery.
