Incident Narrative

On or about 10/18/2015 an employee was attempting to clear a jam in a parcel bundling machine and climbed on top of a rolling wire cage container approximately 41" high x 48" long x 40" wide. The employee slipped from the cage and fell to the floor fracturing their ribs and collarbone.
