Incident Narrative

An employee climbed onto a 6-foot step ladder to place a box onto a metal shelf. As the box was being placed, the shelf slipped out of its post slots striking the employee and causing her to fall approximately 2 to 3 feet from the ladder onto the floor. The employee sustained a concussion and a possible cheek bone fracture.
