Incident Narrative
On September 22, 2015, an employee made a delivery to a customer location. A forklift driver for the customer struck part of the delivery trailer causing damage. When the trailer was unloaded the semi truck operator climbed up onto the trailer to inspect for damage. As the operator was inspecting the trailer he slipped and fell five feet to the ground below. The employee was hospitalized with broken ribs and a collapsed lung.
